HIV clinics brace for crisis as USAID funding ends for antiretroviral drug and other projects

The US government has announced deep cuts to its international aid budget, slashing over 90% of contracts under its key development agency, USAID, and reducing foreign aid spending by US$60 billion. The decision is expected to have devastating consequences, with projections suggesting more than 500,000 deaths in South Africa over the next decade. Additionally, thousands of workers in aid organizations are set to lose their jobs in the coming days.

Among the most critical cuts are funding for HIV programs, ongoing research, clinical trials for potential HIV vaccines, and new preventive medications. South Africa’s Health Minister, Dr. Aaron Motsoaledi, has instructed state-funded clinics to ensure that no patients are left without life-saving treatment.

Aid organizations are now in a race against time, scrambling to secure alternative support for nearly 900,000 HIV patients before funding runs out. “Instead of a careful handover, we're being pushed off a cliff,” said Kate Rees from the Anova Health Institute, one of the largest recipients of US funding for HIV programs.

According to the World Health Organization (WHO), there are nearly 40 million people living with HIV across the world, the majority of whom are women. Of those diagnosed with HIV, approximately 630,000 die from HIV-related causes. There is no cure for HIV and AIDS, but there are medications that can help manage the progression of the disease. In 2023, 77% of people living with HIV were receiving antiretroviral therapy. Fewer people than ever are dying from the disease thanks to these treatments. Yet, President Trump's cut to development funds seriously impacts access to treatment worldwide, which experts fear may provoke an epidemic that we haven't seen since the disease's peak in 2004.
